FACILITY SERVICES

KareFirst provides comprehensive healthcare services to skilled nursing, assisted living, and independent living facilities. We support facilities of all sizes, from communities with fewer than 100 residents to large campuses serving more than 250 residents, in both urban and rural settings.

We deliver enhanced primary care services in collaboration with each facility’s attending physicians and medical directors. In addition, KareFirst offers specialty care programs tailored to the needs of long-term care populations, including dialysis coordination, post-stroke recovery, congestive heart failure and cardiac management, wound care, diabetes management, and psychotropic medication reduction.

KareFirst nurse practitioners function as an integrated extension of the facility’s clinical team, working closely with the Director of Nursing and the Administrator to support the facility’s goals and care initiatives. While residents are our patients, the facility is our client. Through proactive, consistent onsite care, KareFirst helps reduce hospital readmissions, improve patient outcomes, strengthen survey performance, and enhance overall clinical quality. Our presence also serves as a strong marketing advantage by providing a higher level of care delivered more frequently than facilities without KareFirst services.
